Montville High School supports the beliefs that a strong program of extra curricular activities provides a balance of educational programming for its students. Athletic competition contributes to greater student awareness of self-discipline, growth in emotional maturity and in increasing realization of the worth of the individual. Students should learn that participation in athletics is not a right but a privilege. Maintaining one's academic standing in the classroom and maintaining a high standard of conduct outside of school as well.

Registration Forms and Policies

The Montville Athletic Department is excited to utilize FinalForms as our athletic registration platform. FinalForms streamlines the registration process for families while improving communication between student-athletes, parents, coaches, and the athletic department.

All student-athletes must be registered and approved in FinalForms before participating in any athletic activity, including summer workouts, practices, conditioning sessions, tryouts, and interscholastic sports.

Registration Information

Families are encouraged to register for all sports their student-athlete plans to participate in during the school year. If plans change, sport selections can be updated at any time by logging back into FinalForms.

Before beginning registration, please have the following information available:

  • Emergency contact information
  • Insurance information
  • Physician and dental provider information
  • Medical history and health information
  • Current physical examination information

 

Getting Started

  1. Visit montvilleschools-ct.finalforms.com
  2. Create a parent account.
  3. Add your student-athlete.
  4. Select the sport(s) your student-athlete plans to participate in.
  5. Complete and sign all required forms.

 

Physical Examination Requirement

Student-athletes must have a current physical examination on file and complete all required FinalForms paperwork before being approved for participation.
